FDA Grants Breakthrough Therapy Designation to a 5-MeO-DMT Nasal Spray

The US Food and Drug Administration granted Breakthrough Therapy designation to BPL-003, Beckley Psytech's intranasal 5-MeO-DMT, for adults with treatment-resistant depression. It's the first time a US regulator has formally flagged a 5-MeO-DMT product as potentially offering a substantial improvement over existing treatments.
What Breakthrough Therapy designation is
Who qualifies: drugs for serious or life-threatening conditions, where early clinical evidence suggests a substantial improvement over existing therapies on at least one important measure.
What it provides: more intensive guidance and closer collaboration with the FDA during development. It can also speed up both development timelines and regulatory review.
What it isn't: approval. The drug still has to prove itself in large Phase 3 trials, and the FDA can still decline to approve it.
About BPL-003
The product: a patented nasal spray of mebufotenin benzoate (a salt form of 5-MeO-DMT), using a spray device already used in an approved drug product.
Uses being studied: treatment-resistant depression and alcohol use disorder.
Design goal: rapid, lasting effects from a single dose, with a short psychedelic experience.
The evidence behind it
The designation followed the Phase 2b trial results announced in July 2025 (see that post). In that trial, a single 8 mg or 12 mg dose reduced depressive symptoms more than a comparison dose within 24 hours, and the effect lasted through the eight-week trial. Most people were ready to leave the clinic 90 minutes after dosing.
The companies highlighted that this fits the existing two-hour, in-clinic model already used for another approved depression treatment, the esketamine nasal spray (Spravato). At the time, they expected Phase 3 trials to begin in the second quarter of 2026, subject to FDA agreement. In November 2025, the two companies completed their merger to form AtaiBeckley.
What this means, and doesn't
Breakthrough status speeds up the conversation with regulators, but it doesn't guarantee the outcome. BPL-003 remains an investigational drug until it succeeds in Phase 3. Many drugs with this designation are eventually approved, and some aren't.
It's still a real milestone. A molecule long associated with underground toad ceremonies is now on a formal regulatory pathway in the United States.
If approved one day, this would be a medical treatment given in a clinic for a diagnosed condition, quite different from ceremony. The two can be seen as complementary: clinical research building understanding of the molecule, and ceremonial practice holding the human, relational side of the experience.
